Sports Physical Requirements

Team Impact will require all players to have a medical eligibility form on file signed by a MD, DO, nurse practitioner, or physician’s assistant with clinical training to evaluate and determine a player’s eligibility to participate in sports.

What is the purpose of a Sports Physical?

A sports physical is system-based examination to include cardiovascular, nervous system, general medical conditions, respiratory system, gastrointestinal and urogenital, dermatological, musculoskeletal and mental health.

The purpose of the exam is to evaluate the player’s eligibility to participate in sports and determine medical eligibility of the athlete on a broad range of health issues. 

When should the Sports Physical be conducted?

According to the recommendations by the American Academy of Pediatrics, the Pre-Participation Physical Examination (sports physical) should be conducted at least 6 weeks before the first preseason practice to allow time to evaluate the athlete and treat any medical conditions found during the visit. 

Possible results of the Sports Physical

After the examination, the primary care provider can find the athlete:

  • medically eligible for sports without restrictions
  • medically eligible for sports without restriction, but further evaluation needed
  • medically eligible for certain sports listed on the form
  • not medically eligible for any sports, pending further evaluation
  • not medically eligible for any sports

The form submitted to Team Impact must indicate that the player is “medically eligible” to participate in sports.


When is the form due?

The medical eligibility form must be on file prior to the player being able to attend the first practice. The medical eligibility form must be dated less than twelve (12) months prior to the date of the first practice.

The medical eligibility form is included as part of the Pre-participation Physical Evaluation (sports physical) form.

The pre-participation physical evaluation form was developed by the American Academy of Family Physicians, American Academy of Pediatrics, American College of Sports Medicine, American Medical Society for Sports Medicine, American Orthopaedic Society for Sports Medicine, and American Osteopathic Academy of Sports Medicine. This is the same form that the Georgia High School Association uses.

How can I obtain a copy of the required form?

For your convenience, here is a copy of the Pre-participation Physical Evaluation form. This is a four (4) page form: Please complete pages 1-2 and provide to the person that will perform the sports physical exam. Page 3 is a guideline for the physician (if they choose to use).

Page 4 of the pre-participation physical evaluation form is the Medical Eligibility Form. Please have the person performing the exam complete and sign. This is the only page of the pre-participation form that Team Impact must have on file.

Where can I get a sports physical exam for my child?

In addition to your regular physician and walk-in clinics, many of the local Walmart, CVS, and Walgreens stores perform sports physicals.
